Online Long-Term Condition Service

We are excited to work with Suvera, a trusted digital health NHS Partner, to bring this service to you. They already support patients nationwide to make managing your health much easier.

What will you have access to?

As part of this new online long-term condition service Suvera provides, you will have access to a secure patient portal. From here, you can submit information about your health from home.

When you’ve shared information about your health, a dedicated and specialised care team at the practice will review your data. They will then provide clinical support all year round for your condition management.

How will this benefit you as a patient?

By working with Suvera, we hope to improve your experience with the practice with the following benefits:

  • Providing easier and more flexible access to appointments throughout the year.
  • Giving you a more active role in managing your health.
  • Helping you get the medications and treatments you need faster, so you stay in good health for longer.

Submitting information

You will receive a text message inviting you to use the online clinic and a link to our patient portal. To access the patient portal, use your full name and date of birth to log in.

Answer the questions or submit your blood pressure readings through the patient portal. Any information you share is safe and \\secure and won’t be shared with anyone but your GP practice.

Ongoing appointment

A member of the care team will review the information you share and decide what the actions or next steps should be.

If you need an appointment, this will be done on the phone or via video call. In this appointment, you will discuss your long-term conditions, medicines, and other important lifestyle factors like diet and exercise.

Health Visitors

Health visitor’s prime responsibility is for the care and welfare of children under 5.

Travel Vaccines

We offer all vaccines except yellow fever.

Please book with our practice nurse to discuss, preferably 6 weeks before travel. Some vaccines have a charge – we keep this as low as possible.

We also provide private prescriptions for malaria tablets.


To help us offer the appropriate advice, please fill out the online form before coming to see the nurse.

Contraceptive Advice

We provide a full range of contraceptive services including the Pill, Injections, implant, IUCD and IUS fitting and removal.

The Doctors are able to fit coils and implants, both for our patients and for patients from other surgeries in partnership with Umbrella.

Antenatal Care

The Midwife holds a clinic at Kings Norton on Wednesday and Ash Tree on Tuesday.


If you have a positive pregnancy test you can now make your own online self-referral for your maternity care, without needing to see a doctor or nurse at the practice.

Our practice is now registered and listed on the online referral portal, so all you need to do to refer for your maternity care is visit the secure link below and provide the information requested. You’ll need your NHS number and the name of the practice, with which you must be registered.

The information you provide will be used to process your referral by both the community maternity team assigned to our practice and the booking office of your chosen hospital within Birmingham and Solihull. A copy of your referral will also be shared with us, your GP Practice, for information.

This link can also be used to refer a pregnancy on behalf of someone else, providing you have her consent.

Non-urgent advice: Important

You need to search for the GP practice site by the POSTCODE in the drop-down list to complete your SPA portal referral.

Your referral can only be processed based on the information you provide and can delay your care if incorrect information is provided.
